Applying for Oregon Certification

If you are currently certified by the Unified Certification Program (UCP) in your home state and wish to apply for DBE certification in Oregon, you must provide us with the following documentation:

    

DBE Uniform Certification Application submitted in your home state, dated within the last six months.
If the application date is older than six months, a new Oregon application is required.

    

Business federal tax returns for previous three years, all schedules

    

Current Personal Financial Statement (SBA Form 413), signed and dated

    

Current DBE certification letter from your home state that includes the NAICS codes in which you are certified

    

Home state UCP contact information (phone, email) for requesting additional DBE documentation as necessary

    

Appropriate Oregon licenses and registrations demonstrating the ability to perform a commercially useful function in Oregon in accordance with Oregon laws and rules. Oregon License Directory

For each owner claiming disadvantaged status provide:

    

Individual federal tax returns for previous three years, all schedules

    

Driver's license

Applications

Applications are processed only after all required documentation is received.

If your UCP Application is older than six months, you need to submit a new application, which is available in two formats:

  1. Choose a Word format to fill-in, save and print when completed; OR
  2. Choose a PDF format to fill-in and print. You must complete and print the application in one session (there is no save option).

If you anticipate pursing DBE certification in multiple states, we encourage you to maintain electronic copies of information submitted to your home state.
